New Agency Fraud Reporting Checklist

A new Agency Fraud Reporting Checklist has been added under the ‘Report Fraud Here’ tile on the SHARP & SMART homepages:

This checklist is intended to provide agencies with a set of consistent steps to follow to report suspected fraud impacting the statewide systems. Agencies should follow the instructions on the checklist, including retaining a copy of the completed checklist along with any documentation related to the fraud incident at the agency for recordkeeping.
